Abstract:

The Lebanese hospitals accreditation system (LHAS) is a norm that intends to improve healthcare quality management (HQM) in Lebanese public and private hospitals. As such, the Lebanese Ministry of Public Health used the norm LHAS to exert institutional pressure on hospitals along with New Public Management (NPM) and governance by management tools perspectives. The doctoral thesis at hand studied the operationalization of HQM in a Lebanese private hospital through the deployment of the norm LHAS, considered a management tool. The appropriation dynamics were analyzed through the four dimensions of the appropriation theory (De Vaujany & Grimand, 2005; Grimand, 2006): instrumental, socio-political, psycho-cognitive, and symbolic. The study considered the complexity of the norm, the context (hospital), and the implementation process. The deployment process revealed a reciprocal transformation: the norm by the hospital on one side and the hospital by the norm on the other. The study of the appropriation dynamics showed the emergence of tensions and paradoxes linked to HQM inscription in the organizational functioning and the deployment of the norm LHAS. The hospital’s management was aware of these tensions and paradoxes. Henceforth, they elaborated a strategy to regulate them and facilitate the appropriation process that was based on: human resources management techniques, bioethics and mediation practices, and an emphasis on the cultural dimension of the hospital. Our research was positioned as a hermeneutic phenomenological study with an interpretative phenomenological analysis approach, since we were interested in understanding actors’ representations concerning the appropriation of the norm LHAS and HQM. The research studied the appropriation process using a processual approach.
